Office Policies
OFFICE HOURS & APPOINTMENTS
- Monday through Thursday, 7:00 a.m. to 5:30 p.m.
- Friday 7:00 a.m. to 12:00 p.m.
Appointments can be scheduled by calling 920-769-0152.
A 24 hour cancellation policy is in effect. Insurance companies cannot be billed for missed appointments.

BILLING
All charges will be explained at the time of service. As a courtesy, we will assist with your insurance filing. Payment plans for amounts not covered by
insurance can be arranged with our patient accounts representative. We accept Visa, MasterCard, and American Express as payments options.
